L-idocaine acts as a local anesthetic by retroactively blocking the sodium channel and the transmission of the action potential across the nerve fibers. Sensory nerve fibers are blocked earlier than motor nerve fibers, so low doses of l-idocaine can produce selective sensory block. L-idocaine also has the effect of antiarrhythmia, belongs to Ⅰ class B antiarrhythmia drugs. It reduces ventricular rate, shortens action potential time and absolute refractory periods, and prolongs relative refractory periods. L-idocaine acts quickly and lasts for a short time. The addition of epinephrine to l-idocaine can reduce the rate of systemic absorption, thus prolonging the duration of anesthesia. L-idocaine is a first-line drug for the treatment of ventricular arrhythmias caused by acute and severe hemodynamic abnormalities. It is also effective for some supraventricular arrhythmias, such as bypass-mediated supraventricular tachycardia, and acute or parasympathetic atrial fibrillation cardioversion. It is widely used in topical anesthesia of mucosa (e.g. before endotracheal intubation). EMLA cream is used for superficial skin anesthesia before intravenous intubation. To ensure the anesthetic effect, it should be applied to the skin for 45 to 60 minutes and wrapped with an airtight dressing to promote absorption. EMLA is useful for ear venous catheterization in rabbits and for venous catheterization in young dogs and cats.
